Patient Portal Terms of Use

Last Updated: October 4, 2026 · Effective Date: October 4, 2026

About These Terms

These Terms govern your use of the patient portal and telehealth features ("Portal") that your healthcare provider makes available to you through the Sefton platform. Your care, your medical records, and your privacy rights are the responsibility of your provider, who is the covered entity. Sefton operates the Portal on your provider's behalf as its business associate.

1. What the Portal is (and is not)

The Portal lets you do things like view appointments, join telehealth visits, message your care team, review documents your provider shares, and manage intake. The Portal does not provide medical advice, diagnosis, or treatment, and it is not a substitute for your relationship with your provider. All clinical decisions are made by your provider, not by Sefton or the software.

2. Not for emergencies

Do not use the Portal for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. Telehealth and portal messaging are not monitored for emergencies and are not for urgent or emergency care.

3. Your account

  • Keep your login credentials confidential and notify your provider or us promptly of any unauthorized access.
  • The Portal is intended for the patient (or the patient's authorized parent/guardian/representative). You are responsible for activity under your account.

4. Telehealth, recording, and automated documentation

If your provider uses telehealth, your session may be recorded, transcribed, and processed by automated documentation tools, and real-time translation may be provided, only as described in and consented to under the Telehealth Recording, Transcription & Automated Documentation Consent. You may decline recording and still receive care where clinically appropriate, and you may withdraw consent for future sessions. If you speak a language other than English, you have the right to a qualified human interpreter provided by your provider at no cost to you; ask and one will be arranged. Machine translation is not a certified interpreter and is offered only as an additional convenience, never as a substitute for a qualified interpreter.

5. Your health information

Your provider is the covered entity for your records and issues its own Notice of Privacy Practices. Sefton handles your information as your provider's business associate. How your information is handled is described in the Client-Portal Privacy Policy and the Privacy Policy. To exercise your HIPAA rights (access, amendment, restriction, accounting, confidential communications), contact your provider.

6. Acceptable use

Do not misuse the Portal: no unlawful use, no attempts to breach security or access data you are not authorized to access, no uploading of malicious code, and no interference with the service.

7. No warranty; limitation

The Portal is provided "as is." To the maximum extent permitted by law, Sefton disclaims warranties as to the Portal software and is not liable for your provider's clinical care or decisions. Nothing here limits your rights against your provider or any liability that cannot be limited by law.

8. Governing law and disputes

New Jersey law governs these Terms. If you have a problem with the Portal, please contact privacy@sefton.health first so we can try to fix it. Any claim about these Terms may be brought in the state or federal courts located in New Jersey, or in the court where you live if the law gives you that right. There is no arbitration agreement, you keep your right to a jury trial, and nothing here limits any claim you have against your provider.

9. Changes; contact

We may update these Terms; material changes will be communicated by reasonable means. Questions about the Portal: privacy@sefton.health. Questions about your care or records: your provider.
