Evaluations and reports to payers

Generate evaluation reports and progress reports as PDFs, submit them electronically through the clearinghouse or download for manual submission, and track submission status.

Sefton Editorial · Reviewed by Sefton Product ·

Payers ask for two kinds of documents beyond the claim: the evaluation report and the progress report. Both are generated from data you already entered.

Evaluation report

From the evaluation note: Generate Evaluation PDF (built from the appointment and transcription data), Preview, Submit. Choose the payer and the submission method, Electronic or Manual (download). Electronic submissions show up on the evaluation submissions page with their status. Prior authorization requests can reference an existing evaluation or generate a new one from an appointment (Prior authorizations).

Progress report

From Goal Progress Report: pick the date range, Generate PDF, then Print, Submit to Payer, or attach to a claim (Goals and progress).

The Submit Evaluation Report to Payer step with payer and submission method selected.

Attachments on claims

Claim attachments (evaluation, progress report, plan of care, referral) are managed on the claim itself in Billing and are included with the electronic submission where the payer supports it.

Frequently asked questions

Can I attach a progress report to a claim?

Yes. From the Goal Progress Report, generate the PDF and choose the claim; it is attached and travels with the submission.
