Guardians and caregivers

Record parents, legal guardians, spouses, and caregivers on a patient, and choose who is the primary contact, who receives communications, and who signs.

Sefton Editorial · Reviewed by Sefton Product ·

Most pediatric patients arrive with someone. The Guardians and caregivers card on the record makes those adults first-class instead of a free-text note.

Adding a guardian

Add guardian: first and last name, relationship (Parent, Legal guardian, Spouse, Other), phone, email, and preferred language. Then three switches:

  • Primary contact: the main contact for the patient. Only one guardian can be primary; setting it moves it.
  • Receives communications: appointment reminders and patient messages are routed to this person.
  • Legal signer: authorized to sign consents and agreements on the patient’s behalf.
The Add guardian form with relationship and the primary, communications, and legal signer switches.

How the roles are used

  • Portal invitations and reminders go to communications contacts, in their language.
  • Consents (telehealth, intake agreements) are presented to the legal signer.
  • Messages from the portal show which guardian wrote.

Editing and removing

Edit changes details or roles; Remove takes the person off the record (the audit trail keeps the history).
