Plan of care and recertification
Create the plan of care with prescribed frequency and functional goals, record the physician certification, track the 90-day recert clock, and see when delivered visits fall below the plan.
The Plan of care card on the patient record tracks what was prescribed, who certified it, and when it must be recertified.
Creating a plan
New plan of care: discipline (physical therapy, occupational therapy, speech-language pathology, other), start date, frequency (1 to 14 sessions per week), duration, and the functional goals the plan targets. Create plan.
Certification
A new plan shows Awaiting certification. Record certification when the physician signs; that starts the 90-day certification period and sets the recertification due date. Record recertification when the next signature arrives. Expired plans show Cert expired and the Sessions queue warns before the date.

Delivered versus prescribed
The card compares delivered visits with the prescribed frequency and says when you are below plan, which matters for medical-necessity reviews and for the family’s expectations.
Where else the plan is used
- The Good Faith Estimate is prefilled from the plan (Patient payments and statements).
- The portal shows the family a plain-language summary of the plan’s goals, translated.
- Medicare KX-modifier thresholds are computed against delivered services.
Frequently asked questions
Why does the Good Faith Estimate mention the plan of care?
The estimate is prefilled from the plan's prescribed frequency times duration, so the family's expected cost reflects the actual plan.
