Superbills and CMS-1500 forms

Generate a superbill or CMS-1500 from a signed note, batch-generate for every unbilled note, share to the portal, and let families email the form to their insurer.

From one note

Open the signed note (or the visit in Billing) and choose Generate superbill or Generate CMS-1500. The document is built from the note’s codes, the visit’s place of service, and Practice Setup. Review, then Download, Share to portal, or Email.

Batch Claim Generation

Billing > Batch Claim Generation lists every signed session note without a claim. Select all or some and generate CMS-1500s in one pass. “All caught up” means every signed note has its claim.

Batch Claim Generation with signed notes selected and the generate button.

Sharing with the family

Shared forms appear in the portal under Billing > Claim forms and Documents > Shared with you. Families can Download or Email to insurance from there, and see Claim history mirroring your tracking.

Statements and payments

Patient statements and card payments are covered in Patient payments and statements. Good Faith Estimates for self-pay families are on the patient record.

Going electronic

On Sefton Complete the same CMS-1500 data is submitted as an electronic claim through the clearinghouse. See Electronic claims.
