Intake forms
Build questionnaires with short answer, paragraph, multiple choice, and checkbox questions, assign them to patients, and read responses; families complete them in the portal in their language.
Intake forms are questionnaires families complete before or between visits: new patient intake, medical history, developmental history, consent acknowledgements.
Building a form
Intake forms > New intake form. Give it a title and description, then add questions. Each question has a label, a type (Short answer, Paragraph, Multiple choice, Checkbox), and a required switch. Multiple choice questions take a list of options. Save.

Assigning
Assign to patient sends the form to the family’s portal (Forms tab, To do). The patient card shows Intake pending until it is submitted. You can also assign a set of forms at invitation time.
Reading responses
View responses on a completed assignment shows every answer with the question, in the family’s words and translated. Responses are part of the record and are included in the full record export.
Status
Each form shows Pending, Completed, Not assigned, or Inactive. Deactivate and Reactivate control whether the form can be assigned.
Frequently asked questions
Are the questions translated for the family?
Yes. Form titles, descriptions, question labels, and multiple-choice options are shown in the family's portal language. Answers reach you as written, with a translation.
Can I stop using a form without deleting it?
Deactivate hides it from new assignments and keeps every existing response. Reactivate brings it back.
