Sending the invitation

From the patient card or profile, click Invite to Portal (or Invite Patient from the directory to create the record and the invite together).

  1. Patient Details: email, first name, last name, date of birth.
  2. Insurance: select the carrier and enter the subscriber or member ID from the card, or mark self-pay.
  3. Personal Message (optional), in your words; the email itself is sent in the family’s preferred language.
  4. Save and Invite.
The Invite Patient form with patient details, insurance carrier, and a personal message.

What the family does

The email contains a secure link. The family confirms their details, sets a password, and completes any intake forms you assigned. Their account is created at that moment; until then the directory shows Intake pending.

After acceptance

The family can sign in at sefton.health/patient/login and, on iPhone, in the Sefton app. Their portal shows appointments, goals, home programs, messages, forms, bills, and shared records, in their language. Send them Patient portal: signing in.

Resending

Continue intake on the patient card resends the invitation or lets the office complete the intake on the family’s behalf.
