Patient portal

Patient portal: signing in

For patients and families: accept your provider's invitation, sign in with your password and a verification code, add a passkey, reset a forgotten password, and use the iPhone app.

Sefton Editorial · Reviewed by Sefton Product ·

The patient portal is where you see your appointments, goals, home programs, messages, forms, bills, and shared records. It is in your language.

First time: the invitation

Your provider sends an email with a secure link. Open it, confirm your details, choose a password, and complete any forms they attached. Your account is created when you finish. If the link expired, ask the office to resend it.

Signing in

Go to sefton.health/patient/login. Enter your email and password, then the six-digit verification code. If the office set up a different sign-in for you, follow the instructions in the invitation.

Passkey

In Settings, Add a passkey lets you sign in with your face, fingerprint, or device PIN instead of your password, even after logging out. Remove it any time.

Forgot password

Forgot password on the sign-in screen emails you a link. The link opens a form to choose a new password.

Locked out

Too many wrong codes lock the account for a short time. Wait, then try again; if you are still stuck, the office can help.

iPhone

The Sefton app on the App Store has the same portal. Sign in with the same email and password.

Frequently asked questions

I never got an invitation. Can I sign up myself?

No. Portal accounts are created only from a provider's invitation so that your record is tied to you. Ask the office to send or resend the invitation.

Where is the verification code sent?

To the email on your account, or to your authenticator app if you set one up. Enter the six digits on the screen after your password.
