Patient portal: forms
For patients and families: complete the intake and assessment forms your provider sends, in your language, and review the answers you already submitted.
Forms has two sections: To do and Completed.
Filling out a form
Fill out opens the questions in your language. Required questions are marked; if you skip one, the form highlights it before it sends. Send when done; you see Form sent and the answers move to Completed.
Reviewing answers
View answers on a completed form shows what you submitted. If something needs changing, tell the office in Messages and they can reopen or assign a new form.
